If you have PCOS and have been trying to conceive for a long time, you may have heard a doctor mention a procedure called ovarian drilling. The name sounds alarming, and many women are understandably afraid of being operated on, especially when they are already tired of treatment.

This guide explains what ovarian drilling is, when it may be considered, what the evidence suggests, what the risks are and how to decide with your doctor. It is not an advertisement for the procedure, and it does not mean that you will need it.

The Short Answer

What to Know Ovarian drilling is a keyhole surgery in which small, controlled points are made on the ovary, which in some women with PCOS may help ovulation return. It is usually considered only after simpler treatments have not worked, and it carries surgical risks, including a possible effect on ovarian reserve. Whether it suits you depends on your age, tests and goals, and you can ask your doctor for the reasoning.

Why PCOS Can Make Ovulation Difficult

PCOS is a hormonal condition that often causes irregular or absent ovulation. This can make it harder to conceive, because an egg is not released regularly. For an overview, see PCOD vs PCOS: why the name keeps changing and PCOS and IVF: can you actually get pregnant with PCOS.

Many women with PCOS conceive with lifestyle support, ovulation tablets or injections, IUI or IVF. A small number do not respond to the usual medicines, and this is the group in which doctors may talk about other options. If you have lean PCOS, see lean PCOS: can you have PCOS without weight gain.

What Ovarian Drilling Is

Ovarian drilling, usually called laparoscopic ovarian drilling, is a minor keyhole operation done under general anaesthesia. Through small cuts in the abdomen, a surgeon uses a thin camera and a fine instrument to make a few small marks on the surface of the ovary, using heat or energy.

The idea is that this changes the local hormone environment in the ovary, which in some women can allow ovulation to resume. The exact reason it works is not fully understood, and it does not work for everyone. The effect may be temporary.

When It May Be Considered

Doctors usually reserve this procedure for specific situations. They may consider it when:

  • Ovulation has not occurred with medicines — after trying standard ovulation tablets for an adequate time under supervision.
  • Ovulation medicines are not suitable or not tolerated — for medical reasons your doctor will explain.
  • A laparoscopy is already planned — for another reason, such as checking the pelvis or tubes.
  • Monitoring injections is difficult — for practical reasons, or when the risk of over-response is a concern.

It is not usually the first treatment, and it is not a replacement for IVF when IVF is needed for other reasons. See fertility tablets for women and fertility medicines and injections explained for how simpler treatments work.

What the Evidence Suggests

Studies have shown that ovarian drilling can restore ovulation in some women with PCOS, and in some it has led to pregnancy. Compared with other treatments, the results have been similar in some studies and uncertain in others, and benefits vary from woman to woman.

Please be cautious about any claim that drilling "cures" PCOS or guarantees a pregnancy. It does not treat the underlying condition. Read the truth about IVF success rates to see how to judge such claims.

Risks and Downsides

As with any surgery, ovarian drilling carries risks. These include risks of anaesthesia, infection, bleeding and, rarely, injury to nearby organs. Scarring around the ovary and tubes is another concern, since adhesions could affect fertility.

An important point is the possible effect on ovarian reserve. Because the procedure acts on the ovary, there is concern that too much treatment could reduce egg numbers. Your doctor should discuss this openly, especially if you are older or already have a low reserve. Read AMH levels by age and ovarian reserve testing explained.

How It Compares With Other Options

There is no single best path. A doctor may weigh several options together with you.

  • Lifestyle support — gentle changes in routine and sleep, without pressure. See lean PCOS and weight and fertility.
  • Ovulation medicines — tablets, with scans to monitor. See follicular study explained.
  • Injectable medicines with IUI — more intensive monitoring, with a risk of too many follicles.
  • IVF — useful when other factors are present or time matters. See the IVF process step by step.
  • Ovarian drilling — a surgical option in selected cases.

If you want to understand the risk of over-response with injections, read ovarian hyperstimulation syndrome.

What Recovery Looks Like

Most women go home the same day or soon after, and recover within a short time, though it depends on the person. You may feel some discomfort in the abdomen or shoulder, which is common after keyhole surgery. Your doctor will give you instructions on rest, medicines and when to return to normal activities.

Please contact your doctor if you have fever, severe pain, heavy bleeding, vomiting or any symptom that worries you. Do not take any medicine on your own, and do not begin fertility medicines without guidance.

What Happens Afterwards

After the procedure, doctors usually monitor whether ovulation returns, using scans or other checks. Some women ovulate and conceive naturally, while others still need medicines. If ovulation returns, the effect may last for some time, though it can fade.

If no pregnancy occurs after a reasonable period, your doctor may suggest moving to the next step, such as IUI or IVF. Please do not feel that this means the procedure failed or that you did something wrong. Read IUI vs IVF.

Questions to Ask Your Doctor

  • Why do you suggest ovarian drilling in my case?
  • What have we tried so far, and for how long?
  • How might it affect my ovarian reserve, given my age and tests?
  • What are the risks and the chance of scarring?
  • What is the plan if I do not ovulate or conceive afterwards?
  • Would IVF or another option be reasonable instead?
  • How long would we wait before moving on?

Emotional Side

Facing surgery after months or years of treatment can bring fear and exhaustion. It is also common to feel worried about doing "everything" and still not getting an answer. Please be kind to yourself, and share the decision with your partner.

Common Myths About Ovarian Drilling

  • Myth: "Ovarian drilling cures PCOS." — It does not cure the condition. In some women, it may help ovulation return for a time.
  • Myth: "Everyone with PCOS needs it." — Most women with PCOS do not. It is considered only in selected situations.
  • Myth: "It is a risk-free minor procedure." — It is keyhole surgery with anaesthesia, and it carries risks that should be discussed.
  • Myth: "After drilling, you will definitely conceive naturally." — Some women do, and some still need other treatment. No pregnancy can be guaranteed.
  • Myth: "A herbal or home remedy can do the same thing." — No such remedy is reliably proven. Please speak with your doctor, and never start or stop a medicine on your own.

When to See a Doctor

If you have PCOS and have been trying to conceive without success, ask your doctor to review your plan, including how many cycles you have tried on each medicine. If surgery has been suggested, ask for a clear explanation and consider a second opinion.

Seek urgent care if you have severe pain, heavy bleeding or fever, especially after any procedure. You can also speak with one of our specialists about your options.
